Employment Application Process

Pre-Employment Forms

  • Applicant Release Form – the information on this form is used to request background checks
  • A completed and passed Drug test is required before employment can begin
    • In Forsyth County:
      • Drug Test Form-this form must be printed and presented to a certified lab associated with Coe Management, LLC.
    • Outside of Forsyth County:
      • Drug Test Form: this form will go to Coe management and they will send candidates for employment a list of local laboratories via email.
        • Company Name, “Moravian Church in America, Southern Province”

Results will be returned to the Provincial Financial Office and the local hiring manager.

New Employee Enrollment Forms

All the information on the following forms is required in order to enter the new employee in the payroll system. Incomplete forms will delay the processing of payroll checks. All completed forms must be sent to [email protected] before the date of employment.

    1. Moravian Church, Southern Province Application Form
    2. Employee Information Form – please include the date of employment.
