Pre-Employment Forms
- Applicant Release Form – the information on this form is used to request background checks
- A completed and passed Drug test is required before employment can begin
- In Forsyth County:
- Drug Test Form-this form must be printed and presented to a certified lab associated with Coe Management, LLC.
- Outside of Forsyth County:
- Drug Test Form: this form will go to Coe management and they will send candidates for employment a list of local laboratories via email.
- Company Name, “Moravian Church in America, Southern Province”
- Drug Test Form: this form will go to Coe management and they will send candidates for employment a list of local laboratories via email.
- In Forsyth County:
Results will be returned to the Provincial Financial Office and the local hiring manager.
New Employee Enrollment Forms
All the information on the following forms is required in order to enter the new employee in the payroll system. Incomplete forms will delay the processing of payroll checks. All completed forms must be sent to [email protected] before the date of employment.
- Moravian Church, Southern Province Application Form
- Employee Information Form – please include the date of employment.
- Federal W-4 withholding form.
- North Carolina NC-4 withholding form.
- Eligibility to Work I-9 form. Two forms of identification must be copied and included with the I-9 form. The person at the church witnessing and copying the id is responsible for completing Section 2 of that form. The employee completes Section 1.
- Direct Deposit – the employee has to sign up for direct deposit.
- The drug tests are no longer coordinated by this office. All Pre-Employment / Safe Sanctuary Drug Tests should be set up by the person/church/school requesting the test. Click here to find the online request form. Fill out the form for the applicant via the link provided and then the applicant should be instructed go to the location below for the test. The results will be emailed back to the requester and HR as well. Drug tests are done at Coe Management Group at the address below:
